The process starts with an idea.  This can be conveyed by words or by sketch. 

Example: "we want the bride and groom standing with our two dogs, pulling on the dress & the pants of the tux."

Example: "we want the bride & groom dressed in swimming suits about to dive off a board.The bride has a veil, the groom has a top hat."

Many decisions are made in the beginning stages. If the groom were into basketball "would he be dribbling, or shooting or standing holding the ball? Would he be wearing a tux or basketball uniform?" The answers to this questions will be used to make the cake topper.

An armature (or skeleton) is made of aluminum wire, and aluminum foil and firmly attached to the base. It must be firmly fixed so the sculpture can be held upside down without falling off. 

All sculptures must be "hollow" in the thick areas. It is desirable to have a thin even layer of clay covering only the outside. This applies to all sculpture - even bronze. If there is uneven thickness then the outer perimeter cannot be fired or cast evenly.
